The Team

Milford Regional has assembled a highly experienced team of medical professionals for the FreshStart weight loss surgery program that includes a bariatric dietitian, bariatric nurse practitioner, behavioral health counselors, physical therapist and bariatric surgeons from UMass Memorial Medical Center, a recognized leader in bariatric surgery. Individuals in this program benefit from this team approach that is comprehensive and personalized.

Program Coordinator

Colleen Berg is the program director for our surgical weight loss programColleen Berg, MSN, DNP, NP-C, CBN, CPN earned a bachelor of science degree in nursing at Stonehill College, Easton, MA in 1986. She continued her training at Massachusetts College of Pharmacy and Health Sciences, graduating with a master of science degree in nursing, and attended Regis College, Weston, MA, completing a doctor of nursing practice program.

Colleen has been working as a family nurse practitioner since 1999, most recently at Southcoast Center for Weight Loss/Wareham Surgical Associates, Wareham, MA. She has been a clinical nursing instructor at Curry College, Milton, MA and Bristol Community College, Fall River, MA. She is a certified family nurse practitioner, pediatric nurse and bariatric nurse.

Board-certified Surgeons

Philip Cohen, MDPhilip A. Cohen, MD graduated with a medical degree from Boston University School of Medicine, Boston, MA in 2001. He continued his training at Boston Medical Center with a surgical residency program and a surgical research fellowship. During his surgical residency, Dr. Cohen was the recipient of the senior resident teaching award for two consecutive years. In addition, Dr. Cohen performed an advanced minimally invasive surgical fellowship at the University of Massachusetts Memorial Medical Center.

Dr. Cohen is a member of the surgical medical staff at University of Massachusetts Medical Center and he is an Assistant Professor of Surgery at UMass Medical School.

Donald Czerniach, MDDonald Czerniach, MD, FACS received his degree in medicine from the University of Medicine and Dentistry of New Jersey, Newark, NJ in 1995. He performed his surgical residency at George Washington University Hospital, Washington DC where he was chief resident. Continuing his training at the University of Massachusetts Memorial Medical Center, Dr. Czerniach completed a research fellowship and clinical fellowship in advanced minimally invasive surgery.

Dr. Czerniach is an Assistant Professor of Surgery at the University of Massachusetts Medical School and he is the Director of Minimally Invasive Surgery at UMass Memorial Medical Center.

John Kelly, MDJohn J. Kelly, MD, FACS earned his medical degree from the University of Massachusetts Medical School, Worcester, MA in 1992. He completed his internship and residency in general surgery at UMass Medical Center where he was chief resident. Dr. Kelly continued his training at UMass with a fellowship in laparoscopic surgery.

Dr. Kelly is an Associate Professor and is the Director of the Laparoscopic Surgery Fellowship program at UMass Medical School. He is also the Director of Bariatric Surgery at UMass Memorial Medical Center. Dr. Kelly is an appointed member of the Massachusetts Department of Public Health Weight Loss Surgery Panel and the coordinator of the surgical task force.

Richard Perugini, MDRichard A. Perugini, MD graduated from the University of Connecticut Medical School, Farmington, CT in 1994. He completed all of his surgical training at the University of Massachusetts Medical School including his internship and residency in general surgery, and fellowship in minimally invasive surgery. During his surgical training, Dr. Perugini performed a research fellowship and he was the recipient of a resident teaching award and the H. Brownell Wheeler Award.

Dr. Perugini is an Assistant Professor of Surgery at UMass Medical School and he is the Surgical Director at the Weight Center at UMass Memorial Medical Center.


Patients should call to pre-register up to two weeks in advance for any scheduled surgery and out-patient appointment including most diagnostic tests, lab work and pre-operative testing. This eliminates the need to wait for an admissions representative upon arrival. Patients can go directly to their point of service. Admissions representatives are available to take calls from 7 a.m. to 6 p.m., Monday through Friday.
